Health and Wellbeing Coordinator
The successful candidate will be required to develop, implement and manage a programme of social prescribing initiatives designed to improve student’s individual development, mental wellbeing, physical wellbeing and economic development.
The ideal candidate will work in partnership with the Student Management Team (HWS Activators) who organise and deliver the HWS student activity sessions, to ensure appropriate exit routes are established for targeted students. They will also provide a central point of contact for National Governing Bodies, Active Partnerships, community groups and local sports clubs and represent the College on relevant groups where appropriate.

This role is an exciting addition to our Social Prescribing team. We are looking for an innovative, motivated Health Coach to join the Medway Peninsula Primary Care Network (PCN); helping to strengthen, develop and support local general practice, delivering high-quality care to local people.
You will deliver group and one-to-one sessions on healthy eating, physical activity, and health and wellbeing, in line with personalised care plans; engaging with patients to support them to take an active role in their health care, and providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team in line with the strategic objectives of the PCN.
Applicants must have relevant personal or professional experience of working with vulnerable adults and mental health in the social care, health, education or voluntary and community sector; and ideally experience of working in a health environment, GP Surgery, Health Centre or Hospital. You must be able to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
