This session was the 3rd face to face follow up for a 42-year-old female client who had been experiencing back pain in Lx and Tx.

At the beginning of the session, the client described how they have had a decrease in pain since the last session 2 weeks ago. After this session, the client was provided with several exercises including book under bum, knee hugs, child’s pose, bird dog and pelvic tilts. The client had felt a large decrease in pain since completing these exercises and no longer had any complaints of Tx pain. The client now only described a ‘slight niggle’ on the left side of the lower back which comes and goes.

After this conversation with the client, they were then assessed for their ROM at the hip and Lx. Lx flexion, side flexion and rotation experienced no pain, however there was still some pain in Lx extension. Tx flexion, extension and rotation were also assessed with no pain present, showing a good amount of progress since the previous session. Hip flexion had shown no pain however extension shown some pain in the left-hand side of the lower back. Overall, the client’s ROM had improved significantly with no pain present.

The client had mentioned their use of an abdominal roller when at home to help strengthen their core muscles, however they have been struggling to complete the exercise. This was a good opportunity to introduce the client to the gym next to the clinic. The client does not currently attend a gym however they are enthusiastic to learn more exercises. The client spent approximately 10 minutes of the session being shown how to complete the abdominal roller exercise. They were advised to begin with short and smaller movements to then be able to increase the distance that they roll as they increase their core muscles. This is an exercise and an experience that the client really enjoyed, so will be important to incorporate this into further sessions.

The aim of this session was to assess the clients progress and identify if they need to be provided with further exercises, progressions or regressions of the exercises that have already been provided. As the client had been experiencing less pain and had been completing all exercises at home, they were provided with more exercises to complete at home. These exercises included dead bugs, side lying abductions and glute bridges. As the client mentioned they had felt a lot of tightness within the glute while performing the side lying abductions, they were also provided with a piriformis stretch which they said had helped with this. They were also provided with body weight squats which were performed seated with a bolster/pillow on the chair to help aid the client when knowing how far to complete the squat. This can then be progressed to a full body weight squat and then progressed further to a weighted squat. All exercises were prescribed with 3 sets and 8-10 reps to begin with.

The next session with this client is booked 2 weeks from the date of this session. This will be a good way of assessing the clients progress and to begin incorporating some sessions within the gym. As the client is very enthusiastic to learn these new exercises, it will be important to keep the session engaging and exciting for them by making use of the equipment within the gym.
