Within this follow up session with a client who first came with lower back and thoracic pain, a gym-based treatment session was provided.

To begin with, the client had reported no pain at all within the Lx or Tx of the back, however they did report some 24-hour knee pain (5/10). Upon assessment of the right knee, there was some slight inflammation however the client did not experience any pain when walking or during any movements at the knee joint. The client was advised to apply cryotherapy treatment after the session to help reduce some of the inflammation and pain. This will then be assessed again during the next session.

The client was also assessed for movements at the hip, Lx and Tx with no pain present however there was reports of a “slight niggle” (4/10) at the left-hand side of Lx during Lx extension and side flexion.

The rest of the session was gym based, which was planned to begin on the cross trainer for approx. 10-15 mins of cardio. However, the client was reporting that this was becoming increasingly uncomfortable at their knee after 4 mins, so this was then moved to the exercise bikes to help relieve the pressure and loading through the knee where no pain or discomfort was reported for the rest of the session.

The next exercise was focused on the upper body with biceps and triceps being the focus of this. The exercise was performed on the cable machine with bicep curls performed on one side and triceps pull downs performed on the other side for 3 sets of 8 reps on each side with 30 seconds of rest between each set. This was to help increased upper body strength which was a goal that the client had set for themselves and is enthusiastic to achieve during the sessions.

The next two exercises were performed on the smith machine which helped to support the client throughout the movements. The first exercise was the shoulder press for 3 sets and 8 reps, which the client had found challenging but was enthusiastic to complete. The next exercise were back squats where the client also found challenging however by providing a bench behind them so that they can feel more supported and more confident knowing that they can perform the squat into more of a seated position. This also allowed them to improve upon technique. There was no addition weight added to the bar for each exercise.

For the remainder of the session, a cool down was provided with a light bit of core work involved. Dead bugs were completed for overall 1 min followed by stretches at the hamstrings, adductors, piriformis, quads, chest, and triceps. This was important to help reduce heart rate to a neutral rate as well as to help reduce injury.

For the next session in 1 weeks’ time, the focus will be aimed towards the lower body. The aim of this will also be to improve upon time management to guarantee a smoother session with more confidence during delivery.
