13/11/2020-Marjon Clinic- 3hrs

First hour:

For the first hour, I saw a 58 y/o female client c/o neck, back, shoulder, elbow, and wrist P. I have previously seen this client and they have had no changes in their response to the covid questionnaire, no changes in medical history since the last I saw them, however, they have had changes in P. They are now experiencing elbow P 8/10 VAS, neck P 6/10 VAS with EMS, and shoulder P 7/10 VAS.

Unfortunately, I took some time off last week due to illness which meant this client saw a different therapist and this is when the P started. The therapist correctly gave the client HBE’s to do before coming to see me this week which would help to decrease pain in the new areas that the client was experiencing however, one exercise was increasing pain in the wrist for this client which was wrist flexion/extension with a can of beans in their hand. This client felt it difficult to hold the can of beans as not only was it too heavy but too big. The therapist that prescribed these exercises did not give any regressions or progressions with this so the client continued to do the exercise every day until seeing me today.

I told the client that it may be better to do the exercise with something smaller and lighter to work their way up and help to decrease pain, increase support, strength, and stability in the wrist. I suggested that the client use resistance bands starting this week, using the lightest one for this week and when they return next week (20/11/2020) we can adjust to suit how the client feels. The client also felt it difficult to do these exercises off the edge of a table or chair for example as they were previously told to do therefore I showed them a way to perform this exercise of their knee with a resistance band so they knew exactly what they had to do while at home as well as running through all the other exercises they had been doing as part of their HME program to ensure they were performing them correctly and effectively and had to correct a few movements as the client had forgotten the ways they were to correctly be performed and therefore they were slightly adapted and not working the targetted areas as efficiently as the correct technique would.

Second and Third hours:

For the second and third hours, I went over my notes, updated the client’s HME program and sent them the revised version, and began research for our research project to be presented over the next few weeks in spare time between clients in the clinic.

Thoughts on today’s session:

Today’s session went really well in my own personal opinion, I was able to identify incorrect technique of rehabilitation exercises, correct them and show the client where they were going wrong in the correct manner to not make the client feel uncomfortable. Alongside this, I was able to identify a regression of an exercise that will help the client and hopefully make them feel better when they come for their session next week. I don’t feel as though I have any improvements based on this session as I am confident and proud of my performance.

Reflecting back on the session:

I saw this client again today (20/11/2020) and as assumed, over the week they have improved in grip strength and the exercises have helped in decreasing pain for this client, we are going to continue to have regular appointments and ensure that we keep progressing for future sessions. However, I am happy with the way the client is currently progressing.
