Background:

27 y/o male c/o P in R shoulder from the top (acromion) to deltoid tuberosity. 6/10 VAS all day when moving at work (mechanic, R-dominant). Previous AC sprain in R, groin injury R, knee injury L. Not been training/exercising since lockdown. Agg movements above 90 degree and flexion 70-120 degrees, shoulder press hurts. Easing while at rest.

Clinical impression:

  • RSI
  • Supraspinatus impingements
  • Tight deltoids

Rx: Treatment during the session:

  • Massage of the back and shoulders
  • Stretching and strengthening exercises
  • Resistance bands:
    • Row (seated)
    • W’s (standing)
    • Flexion
    • Flexion at 90 degree and abduction (horizontal)
    • Extension and abduction
  • Stretching: done during the session and as a take home activity when player can.
    • Wall stretch
    • Childs pose.
    • Downward dog
    • Thread the needle

Reflection of the session:

This session went well, the player was very responsive to treatment and noticed an immediate impact of treatment decreasing pain and increasing range of motion. I was not confident in the treatment I was giving and therefore a few days after when I saw my supervisor again, I mentioned what was done during the session, the findings and my clinical impression to gage their thoughts on the treatment I had given, and they agreed on my diagnosis and DDx and stated the treatment was good and we worked together to develop a plan for this player during future training sessions. This will allow the player to continue training with the team when in the gym sessions allowing them to be part of the team but working on decreased weight exercises building strength and stability within the training setting.
